Current Parents
Dear Parent(s) of incoming 2008-2009 students. The following items are required to complete and keep current your child's school file:
2008-2009 Emergency Contact Sheet, Birth Certificate (copy),Verification of Receipt of the Department of Children and Families Pamphlet "Know Your Child Care Center", signed Discipline Policy Form, signed photographic release agreement, current Blue Immunization Record (DOH Form 680), and a current Yellow Health Exam Record (DOH Form 3040).
The pamphlet, physical forms, immunization forms, and emergency contact sheets are required by the Department of Children and Families and the school must be in receipt of all of the items listed above prior to your child's start date.
If your child requires a doctor's appointment to complete a physical or immunization, and the paperwork will be delayed past the due date, please let the school office know the date of the scheduled appointment.
