The
pamphlet, physical forms, immunization forms, and emergency contact
sheets are required by the Department of Children and Families and the
school must be in receipt of all of the items listed above prior to
your child's start date. If your child requires a doctor's appointment
to complete a physical
or immunization, and the paperwork will be
delayed past the due date, please let the school office know the date
of the scheduled appointment. Additionally,
if you are a new toddler parent, please turn in the Discovery Toddler
Community Questionnaire, and if you are new Pre-Primary parents, please
turn in the Social Screening Form.
Please
submit these items to the school office as soon as is convenient. Thank
you for your cooperation, and don't hesitate to call (247-4577) if you
