The following items are required to complete your child's school file.

• Emergency Contact/AlertNow
• Birth Certificate (copy)
• Verification of Receipt of the Department of Children and Families Pamphlet
  "Know Your Child Care Center"
• Verification of Receipt of the Department of Children and Families Pamphlet "'The Flu' A Guide for Parents"

• Signed Discipline Policy Form

• Signed photographic release agreement

• Current Blue Immunization Record (DOH Form 680)
• Current Yellow Health Exam Record (DOH Form 3040) 

The pamphlet, physical forms, immunization forms, and emergency contact sheets are required by the Department of Children and Families and the school must be in receipt of all of the items listed above prior to your child's start date. If your child requires a doctor's appointment to complete a physical or immunization, and the paperwork will be delayed past the due date, please let the school office know the date of the scheduled appointment. Additionally, if you are a new toddler parent, please turn in the Discovery Toddler Community Questionnaire, and if you are new Pre-Primary parents, please turn in the Social Screening Form.

Please submit these items to the school office as soon as is convenient. Thank you for your cooperation, and don't hesitate to call (247-4577) if you have any questions regarding these requirements.

 Discovery Montessori School requires that any/all students needing medication administered during the school day on school grounds, must fill out this form and turn it into the office:
